Federal regulations require this information from all driver applicants.
Notice: Per 49 CFR Part 382, all applicants must answer the following questions. Providing false information is grounds for immediate disqualification and may result in criminal penalties.
Your signature authorizes MZK Transport Inc. to query the FMCSA Clearinghouse.
I hereby provide consent to MZK Transport Inc. to conduct a limited query of the FMCSA Commercial Driver's License Drug and Alcohol Clearinghouse to determine whether drug or alcohol violation information about me exists in the Clearinghouse.
I understand this consent shall remain on file and shall serve as ongoing consent for MZK Transport Inc. to conduct multiple limited queries of the Clearinghouse at any time during my employment or contract period without asking me for additional consent.
I understand that if I refuse to provide consent for MZK Transport Inc. to conduct a limited query of the Clearinghouse, MZK Transport Inc. is required to prohibit me from performing safety-sensitive functions, including operating a commercial motor vehicle.
I understand that if the limited query conducted by MZK Transport Inc. indicates that drug or alcohol information exists about me in the Clearinghouse, the FMCSA will not disclose that information to MZK Transport Inc. unless I give additional specific consent within the Clearinghouse.
However, I understand that MZK Transport Inc. will be required to conduct a full query of the Clearinghouse within 24 hours after a limited query indicates that drug or alcohol information exists and that if I do not grant consent within the Clearinghouse for that full query I will be removed from performing safety-sensitive functions, including operating a commercial motor vehicle.

FMCSA Pre-Employment Screening Program authorization required by federal regulations.
In connection with my application for employment/contract with MZK Transport Inc., I understand that consumer reports and/or investigative consumer reports may be obtained. These reports may contain information about my driving record from the FMCSA Pre-Employment Screening Program (PSP), including: crash data for the previous 5 years, roadside inspection data for the previous 3 years.
Authorization: I hereby authorize MZK Transport Inc. and its agents to obtain information from the FMCSA Pre-Employment Screening Program (PSP) regarding my commercial driving record. I understand that I have the right to request a copy of any report obtained. I understand that the information obtained will be used to evaluate my suitability for employment/contract as a commercial motor vehicle driver.

Consumer background check authorization required for employment consideration.
In connection with my application for employment with MZK Transport Inc., I understand that investigative consumer reports may be obtained. These reports may include information as to my character, general reputation, personal characteristics, mode of living, and may include criminal records, employment verification, education verification, and other public records.
I hereby authorize MZK Transport Inc. and/or its designated agents and representatives to conduct a comprehensive review of my background through a consumer report and/or an investigative consumer report to be generated for employment and/or contracting purposes. I understand that the scope of the consumer report/investigative consumer report may include, but is not limited to, the following areas: verification of Social Security number; credit history; current and previous residences; employment history; education background; character references; criminal history records from any criminal justice agency; motor vehicle records; and any other public records.
I understand that I have the right to request a written summary of the nature and scope of any investigative consumer report. I understand that I have the right to request disclosure of the nature and scope of any consumer report. I authorize MZK Transport Inc. to act as my agent for the purpose of obtaining these consumer reports.

This certifies that this application was completed by me, and that all entries on it and information in it are true and complete to the best of my knowledge.
I understand that if I am offered employment, I will be required to provide proof of my identity and legal authorization to work in the United States, and that federal law requires all employers to verify the employment eligibility of all employees.
I authorize MZK Transport Inc. to make such investigations and inquiries of my personal, employment, financial or medical history and other related matters as may be necessary in arriving at an employment decision. (Generally, inquiries regarding medical history will be made only if and after a conditional offer of employment has been extended.)
I hereby release employers, schools, health care providers and other persons from all liability in responding to inquiries and releasing information in connection with my application.
In the event of employment, I understand that false or misleading information given in my application or interview(s) may result in discharge. I understand, also, that I am required to abide by all rules and regulations of MZK Transport Inc.
Per Section 391.21 of the Federal Motor Carrier Safety Regulations: I understand that providing false or misleading information or omitting requested information on this application will be grounds for disqualification and may result in termination of employment at any time and may subject me to civil or criminal penalties.
I understand that MZK Transport Inc. may contact former employers to request information about my alcohol and drug testing history and that I authorize those employers to release such information in accordance with 49 CFR 391.23.
This application will remain active for 90 days from the date signed. If I wish to be considered for employment after this period, I must submit a new application.
